How to send a printable form

• Open the form and fill in the highlighted fields.

• Save it to your device.

• Email it to office@st-katherine.org, or print it and bring it to the church office.

Filling in the form on your computer keeps the names easy to read. If you print a blank form and write by hand, please write clearly.

Please note: for a given fast or feast period, there is no need to resubmit names of the living or departed for each service. The lists are kept and read at every service.
