Complete this form for a bride or groom who has been married before. If both have been previously married, use a separate form for each. Then click Submit, and our priest will contact you to review and sign the official form(s).

If any previous marriage has been dissolved, also complete the dissolution section, and include the certificate(s) of death or the original ecclesiastical divorce decree with your application for the Metropolis Marriage License.
